DHS last week announced a timeline for the roll out of Community HealthChoices (CHC) in the remainder of the state in 2020. Starting this summer, DHS will send an introductory flyer to participants identified within the CHC population. View the flyer by clicking HERE. Then in the fall, notices and enrollment packets will be mailed to participants, information on the LIFE program will be sent to potentially eligible participants, and informational sessions will be held for participants to learn about CHC and how to select a CHC-MCO.

DHS will be hosting summits specifically for providers to learn more about CHC. More information regarding times and locations coming soon, but all impacted agencies are encouraged to save these dates on their calendars.
