What’s the goal of Community HealthChoices, also known as CHC?

To enhance access to and improve coordination of medical care.
To create a person-driven, long-term support system in which people have choice, control and access to an array of quality services that provide independence, health, and quality of life.

CHC was implemented in 14 southwestern Pennsylvania counties in January 2018. It will complete its statewide rollout by bringing the program to southeastern Pennsylvania in 2019, and throughout the remainder of the state in 2020.

Last week, the Department of Human Services wrapped up the last of nine educational sessions for providers in the Southeast region. Each session began with an overview of CHC and then broke out into five targeted sessions. There were more than 1,400 attendees.
